Dear Parent,

This questionnaire gives you a chance to comment on Early On. Sometimes Early On is called the Infant/Toddler Program or the Early Intervention Program or Part C. We have been asked by the State to help find out families’ views about Early On through this questionnaire.

Would you help us by filling out this questionnaire? It is your chance to tell us your thoughts and feelings about Early On and about the services your family has received. Your responses will also help improve services for families across Michigan.

The questionnaire is voluntary and confidential. This means that you do not have to fill out the questionnaire, but we will ensure your privacy if you do. If you do not want to answer a question or if a question does not apply to you, please leave it blank and continue to the next question. Even if you complete part of the survey, your views are important.

We never report the answers of individual families to anyone. All of the information we collect will be reported as group results. If you prefer, we can help you complete the questionnaire over the phone if you call us at 1-800-363-7987.

To access the online survey, you will need a Login ID and a Password. Your Login ID consists of the numbers under the bar code on the upper right-hand corner of your survey; please enter ONLY the numbers. Your password is the entire survey code that includs the numbers and the CAPITAL letters.

By completing the survey, you will be eligible for a drawing to win one of twenty (20) $50 e-gift cards. You will have a chance to leave your name and email address at the end of the survey if you elect to be entered into the drawing. By completing the questionnaire, you are agreeing to participate in this study.
